ENROLMENT
 Who may avail of the service?

1. Kinder – 5 years old by June of the current year

2. Grades 1 to 6 – has passed the previous grade level

Requirements:
Kinder – Birth Certificate (NSO)

Grades 1 to 6 – Birth Certificate (NSO) and Form 138-E

Transferees – Birth Certificate (NSO), Form 137-E,138-E and Reading Ability Assessment 

 

RECRUITMENT AND SELECTION FOR QUALIFIED TEACHER-APPLICANTS

 Who may avail of the service?

A licensed teacher by the PRC

 Requirements:

1. CSC Form 212 in two copies with latest 2×2 ID picture

2. Certified photocopy of PRC professional ID

3. Certified photocopy of LET/PBET rating

4. Certified copies of baccalaureate transcript of records

5. Copies of service record, performance rating and school clearance for those with teaching experience

6. Certificates of specialized training, if any

7. Certified copy of Voter’s ID or any proof of residency

8. NBI Clearance

9. Omnibus Certification as to the authenticity and veracity of all documents submitted signed by the applicant
